Nifedipine is a calcium antagonist of the dihydropyridine type. Nifedipine is relatively selective for the blood vessels with far lower effect on hear muscle cells as compared to the smooth muscle surrounding the blood vessels. Nifedipine has therefore no direct effect on heart contractility and impulse conductivity when in normal clinical doses.

PRESCRIBING INFORMATION

Indications:

The prophylaxis of angina, if especially element vasospastic as Prinzmetal’s angina.

The treatment of hypertension.

Dosage:

  • Angina prophylaxis: 1 tablet twice daily.
  • Hypertension: 1 tablet twice daily.

Usage:

Nifedipin T20 retard is administered orally. It should be swallowed whole, not chewed, broken or crushed.

  • Cardiogenic shock.
  • Clinically significant aortic stenosis.
  • Myocardial infraction in the previous one month.
  • Acute attack in chronic stable angina, in unstable angina.
  • Porphyria.

Ankle edema, headache, fatigue, vertigo, facial flush with heat-feeling. Palpitation, tachycardia. Nausea, diarrhea or constipation.

  • Withdraw of ischaemic pain occurs or existing pain worsens shortly after initiating treatment, stop using this drug.
  • Patients with heart failure or significantly impaired left ventricular function.
  • Reduce dose in hepatic impairment, diabetes mellitus.
  • Grapefruit juice should be avoided by patients on nifedipine.
  • Nifedipine may inhibit labour.
  • Do not use for pregnant women unless necessary. A decision should be made to discontinue nursing or to discontinue the drug. Taking into account the importance of the drug to the mother.
  • Patients should be aware of how they react to drug before driving or operating machinery.
